The Canadian healthcare system has long positioned primary care as the entry point for all treatment, assuming that every Canadian can access a family doctor or nurse practitioner. This assumption no longer holds; 6.5 million Canadians are now unattached to a primary care provider, a number that has risen sharply since 2019. In Ontario, the convergence of increasing physician retirements, declining numbers of new graduates entering primary care, and mounting practitioner burnout is creating a profound access crisis.

This exhibit, Understanding the Primary Care Crisis in Ontario, presents a synthesis map and accompanying analysis that illuminate the fractured and inequitable pathways through which patients attempt to access care. Developed through a series of eleven system maps and multiple analytical frameworks, the final synthesis map integrates stakeholder landscapes, story loops, causal loops, iceberg mapping, and adaptive cycle modelling. The map visualises how patients are caught in repeated triage loops, waiting, cascades, and bottlenecks that compromise continuity of care. It also shows how these system dynamics are, accelerating a transition toward collapse, which we identify as the Omega phase of the adaptive cycle.

The exhibit aims to provoke dialogue about the fragility of Canada’s primary care model, the deep myths that sustain it, and the potential for new pathways of reform. By making complexity visible, it seeks to generate systemic insights and highlight leverage points for intervention, such as supporting multidisciplinary teams, restructuring business models, and expanding access bottlenecks. The map invites policymakers, practitioners, and the public to reconsider how care could be designed to be more equitable, resilient, and responsive.
