This workshop introduces AcciMap, a visual mapping method initially developed for accident analysis within safety science. The safety science research community has developed various systemic analysis methods since the 1990s to analyse accidents, incidents, or near misses across a range of domains, including healthcare, transportation, and energy. AcciMap is one of the most widely used methods and helps to illustrate the diverse range of contributing factors existing across different levels of systems, their interactions, and the impact of external factors such as political, cultural, regulatory, financial, and technological circumstances. However, this method has not yet been introduced to the systemic design research community. The workshop introduces the historical development of various accident causation models (e.g. Bad Apple Theory, Domino Model, Swiss Cheese Model, Normal Accident Model, System Safety Model and Resilience Engineering Model) and provides a hands-on opportunity (group mapping activity) to apply AcciMap to a complex system failure.
